Basic Life and AD&D Insurance

As an important part of your financial planning, you are automatically provided with basic life and AD&D insurance to protect you and your family in the event of an accident or death.

  • Coverage is provided at no cost to you
  • AD&D insurance pays specific benefit amounts for a covered accidental bodily injury that causes dismemberment. If death occurs from an accident, 100% of the AD&D benefit would be payable to your beneficiary
  • Benefits are paid to the beneficiary you designate. Please keep your beneficiary information up to date

What is AD&D insurance?

Should you lose your life, sight, hearing, speech, or use of your limb(s) in an accident, AD&D insurance provides additional benefits to help keep your family financially secure. AD&D benefits are paid as a percentage of your coverage amount — from 25% to 100% — depending on the type of loss.

Have you named a beneficiary?

It’s important to designate a beneficiary(ies) to receive the benefit paid by your life insurance policy. As personal circumstances change, it’s important to keep that information up to date. Be sure that you have named a beneficiary for your basic life and AD&D insurance as well as any supplemental life insurance you may elect. During your initial enrollment or annually during open enrollment, you can name or change a beneficiary by logging onto your enrollment record at ehub.aus.com. To add or change a beneficiary at another time, you will need to complete a New York Life Life Insurance Beneficiary Form and return it to the Benefits department secure fax 888-207-1420 or via email to benefits@aus.com.

Voluntary Supplemental Life Insurance

  • Supplemental employee life insurance — You may purchase coverage in $10,000 increments, up to a maximum of 5 times your base pay (not including overtime, bonuses, commissions) or $500,000, whichever is less. At age 70, this benefit reduces to 67% of the original amount.
  • Spouse life insurance — If you purchase supplemental life for yourself, you may purchase coverage in $5,000 increments, up to 50% of your supplemental employee life coverage amount or $100,000, whichever is less. At age 70, this benefit reduces to 67% of the original amount.
  • Your child(ren) — You may purchase coverage of $5,000 or $10,000 if you purchase supplemental life insurance for yourself. Children from birth to 6 months: benefit limited to $500. All amounts are guarantee issue.

Evidence of insurability

Life insurance over a certain amount may require evidence of insurability (EOI). After electing coverage, you will receive more information from New York Life if EOI is required.

Guarantee Issue

If you enroll when first eligible, you can elect coverage up to the guarantee issue ($200,000 for you and $75,000 for your spouse/domestic partner) amount without Evidence of Insurability (EOI), as long as you elect coverage within 31 days of becoming eligible.

If you do not elect supplemental life coverage when first eligible and you want to enroll in the future, you will need to provide Evidence of Insurability for any coverage you elect.
