- Death of your spouse or covered childLoss or addition of other group coverage
- Change in your or your spouse’s work status that affects your benefits (ie, termination or commencement of employment, reduction or increase in hours of employment, a commencement of or return from an unpaid leave of absence, change in worksite affecting coverage).
- Change in residence that affects your eligibility for coverage

What to do if you experience a life event
If you need to make benefit changes as a result of a life event call the Allied Universal Benefits Department at 1-888-670-7106 or send an email to benefits@aus.com. Changes must be made within 31 days of the life event and documentation may be required. The coverage effective date will be the first of the month following receipt of notification, except in the case of birth, adoption, or placement for adoption. In these cases, the effective date of coverage is the date of the event.
